Safety Incidents: Filing a Report (Web)

Filing an incident report is a crucial step in ensuring workplace safety and compliance with regulations. Here's a detailed guide on how to file an incident report using busybusy desktop. If you need to submit a report via the mobile app please check out our article HERE!

Accessing Open Incidents

The Open tab in your Incidents section will only appear if the Safety Sign-Off daily question is enabled. This tab displays injuries reported by employees when clocking out. You can use the employee’s description to help you assess the situation and then decide whether to proceed with filing a report.

Important

If the injury is minor or reported accidentally, you can select Report Not Required to remove it from the Incident Report. Use the three stacked dots next to the date to mark a report not required for a single incident, or check the boxes next to multiple reports and click the Report Not Required button to mark multiple incidents at once. These incidents will still be recorded under the Daily Sign-Off report as processed.

Filing a Report

  1. Click on the File Report button for the incident you’re addressing.

  2. Mark it as either an Injury or Illness and indicate if it's a recordable OSHA incident.

    • If you’re not sure what would count as a recordable incident, please click the link for How do I know if this is a recordable incident? to get some more information.

Required Fields

Regardless of whether the incident is recordable, certain fields must be filled out before submitting the report. These include:

  • Injury date

  • Time the employee began work

  • Activity the employee was engaged in just prior to the incident

  • Description of what happened

  • Body part injured

  • Location of the incident

  • Doctor/Hospital Information

Submitting the Report

  1. Once all required fields are completed, you can hit submit.

  2. The incident will now be reported.

    • Non recordable incidents will only appear in the Reported tab. OSHA recordable incidents will show up in both the Reported tab and the OSHA Form 300 tab.
